At Applied®, we continue to provide you with the tools needed so you can enjoy a productive life – at home and at work. To help you, the company and our benefit providers are committed to offering a wide range of benefits, incentives and support programs that benefit ALL of you – your mind, your body and your wallet. It’s up to you to take advantage of those resources and achieve your goal of living healthy.
In 2019, we continue to reward you for taking proactive actions to improve your whole health. By completing a biometric screening and mycigna.com health risk assessment, you can reduce your annual medical contribution by $300. We continue to provide a certain type 2 diabetes generic medicine with zero copay to help ensure medical compliance. On the dental side, if you are taking advantage of the preventive services, you will be rewarded with a $200 maximum dental benefit increase in the following year. Finally, a Cigna Personal Health Team member is ready to help if you need access to the programs Cigna offers.
As you think about the new calendar year, understand that chronic conditions make up roughly 73% of our spend and over 40% of us have chronic conditions. Make 2019 the year we tackle these conditions together. Chronic conditions are defined by terms like hypertension, diabetes, obesity, asthma, etc. These conditions, for the most part, can be controlled by you with help from your physician, yet we only have 72.4% of our associates and their family members using prescriptions inaccordance with their doctor’s orders. Additionally, only
34% of associates took the time to see their Dr. for a well visit.
This benefits guide summarizes all of your Applied benefits, as well as many of the tools and resources available to you. With a new year of promises made to be kept, I encourage you read over the benefits guide to make sure your 2019 benefits fit you! Changes for 2019
New Tiers for Associate Dental and Vision Contributions
Beginning January 1, 2019, there will be new contribution levels that provide more coverage options for dental and vision plans. Listed below are the new 4 tier options for 2019.
Associate
Associate + Child(ren) Associate + Spouse Associate + Family
Increase Benefit Amount for Basic Life and AD&D Insurance
Effective January 1, 2019, Basic Life and AD&D benefit amount is 1.5 times annual salary up to $100,000 for all eligible associates.
Increase Benefit Amount for Long Term Disability
Effective January 1, 2019, long term disability monthly benefit amount is 60% of covered earnings up to a maximum of $10,000 per month.
Benefit for Part Time Associates
Part time associates who regularly work 30 hours per week will now be eligible for company provided benefits.
Earning Your Wellness Discount on Medical Contributions
To encourage you to get a complete picture of your health, both a health assessment (by Nov 16)
and biometric screening (by Oct 31) must be completed to earn the $300 annual discount on associate medical contributions for 2019.
Visit your Primary Care Physician in Calendar Year 2019 to receive $50 Gift Card
Visit your primary care physician in calendar year 2019 for a qualifying Comprehensive Preventive Exam
and you will be eligible to receive a $50 gift card from Cigna! Your physician can help you identify risk factors that could lead to future health problems and offer expert advice on how to manage them. More information will be sent in 2019 on how this program will work. You must be enrolled in a Cigna Medical plan in 2019,
to participate.
DAW
If you or your doctor chooses for you to receive a brand name drug when a generic drug is available (such
as when the prescription contains a “dispense as written” restriction), you will be responsible for paying the difference between the cost of the brand name drug and the cost of the available generic drug. You will also be responsible for paying the applicable co-insurance amount for your prescription.
Regulatory Compliance
Contribution limits updated for Health Savings Account (HSA) and Flexible Spending Account (FSA). Cigna Medical Plans Out-of-Pocket Maximum updated to comply with Federal regulations.
