Step 5: Select benefit options
You will then be taken through each of your benefit options. To view your plan options, select “Enroll” for yourself and/or
your dependents. Select “Waive” for yourself and/or your dependents to decline coverage.

Step 6: Select your plan
Select the plan you would like by clicking the check mark next to the plan that best fits your needs. Click “Next” to proceed to
the next benefit.

Step 7: Employer paid life/ AD&D, short term disability and long term disability
These benefits will show as view only pages. No enrollment required.
