Member Items Requiring Authorization
Insignia, Grave Markers (Market Price) and Door Knockers ($200)
There are five designs of the insignia available for purchase in three forms of metal, bronze gilt, 10 K gold and 14 K gold. The insignia includes a light blue and white ribbon Cincinnati ribbon. Additional neck or lapel ribbons may also be purchased.
This item requires prior authorization from constituent society secretaries. The item is purchased directly from Citypride Limited but the vendor will not honor any requests without a signed authorization form. The process to order an insignia is as follows:
1. Contact your constituent society secretary and ask him to give you authorization to purchase either a grave marker or insignia.
2. He will provide written documentation to the Society Staff that you are a member in good standing with your constituent society.
3. After receiving authorization from the secretary, a signed authorization form and information about the insignia and grave markers available for purchase will be mailed directly to you from the Society’s headquarters.
4. This authorization form allows you to purchase your grave marker or insignia directly from the manufacturer, Citypride Limited.
