• An Advanced Glucose Meter ($200 value)

• Lancets
• Charger
• Carrying Case

• Diabetes education
• Access to Livongo’s cellular mobile and web-based diabetes management systems and technologies

If you you are enrolled in a a a a non-HMO medical plan you you already know you must call a a a a a a a health advocate before scheduling any outpatient non-emergency CT scans or MRI services In 2019 this list is is is expanded to include:
• Imaging and CT scans • X-rays
• Joint replacement
• Bariatric surgery
• Musculoskeletal IP/OP procedures
• Breast reductions (medically necessary)
Failure to call before receiving the following services will result in a a $200 penalty Emergency Room Tiered Copay
