Enrolling your child(ren) in Boys & Girls Clubs of Greater Scottsdale is an indication that you have read and will comply with the organization’s policies and procedures.
I will read and discuss the handbook provided by Boys & Girls Clubs of Greater Scottsdale with my child(ren).
My child(ren) and I agree to comply with all the provisions of this Parent/Club Member Handbook while my child(ren) is/are enrolled in Boys & Girls Clubs of Greater Scottsdale.
I have been given the opportunity for clarification and ask questions regarding the discipline program and procedures. I understand I may call for more information.
