               Dear Mr. Stelter:

Your Medicare evaluation is enclosed. Please check the information on the client data sheet on
page 5 to make sure it is correct. In most cases, the costs shown in your evaluation are
estimates for the 2021 plan year.

Medicare has not yet said how much next year’s Part B premiums and cost-sharing amounts will
be, and so for these the 2020 costs are shown. The standard Part B premium this year is
$144.60 a month, but it will be slightly higher next year. As you may know, your Part B
premiums will be deducted each month from your Social Security check.

               Your supplemental coverage options

You already have Part A and indicated in an e-mail that you will be signing up for Part B, which
you can do by calling (800) 772-1213 – you have probably already done this. You will get a new
Medicare card within about three weeks after you call, and it should indicate that you already
have Part A and that your Part B coverage will start on January 1, 2021.

When you enroll in your supplemental coverage, you will also be enrolling in Part D. If you
choose a Medicare Advantage plan, for instance, it will include your Part D coverage as part of
its overall benefit package. If you get a Medigap policy, you will need to enroll in a Part D stand-
               alone plan.

In your questionnaire you indicated that you would like to see comparisons of both types of
coverage – 1) Advantage plans and 2) Medigap policies combined with Part D stand-alone
plans. Once you have decided which type of coverage you want, you can take these steps to
enroll in the plan(s) that you’ve chosen:

 If you decide to enroll in either of the Advantage plans compared in your
evaluation, first verify with your doctors’ offices that they are in the plan’s
network. Even though your medical providers are listed in each plan’s directory,
as shown in Appendix A, these directories are sometimes out of date.

To enroll in either of these plans, call its toll-free number shown on pages 2-3 of
the evaluation. Make sure that the person you speak with understands that you
