Tobacco-Free Discount

Employees enrolled in the Associated British Foods
medical plan that make healthy lifestyle choices and
choose to not use tobacco products or products
that deliver nicotine will receive a medical premium
discount of $60 per month.

You will need to attest to whether or not you are
eligible for the discounted medical premium. To be
eligible for the discounted medical premium you
must certify that:

■ You are not a tobacco user, or
■ You do not use tobacco products that deliver
nicotine, or

■ You completed a tobacco cessation program.

For purposes of this attestation, any product
that contains tobacco or delivers nicotine, means
cigarettes, electronic cigarettes, pipes, cigars,
smokeless tobacco or any other tobacco or nicotine
releasing products regardless of the number of
times, frequency, or method of use.

If you do not disclose your status as it relates to
using tobacco products or products that deliver
nicotine you will not be eligible for the discounted
medical premium.
