Annual Deductible Medical Plan
The amount of medical charges you are responsible for paying before Deinitions
the medical plan begins to share the cost of your expenses. The
deductible is a calendar year deductible. Once satisied, you don’t have
to pay it again until the next year.
Coinsurance
The percentage of a medical bill that the plan pays after you satisfy
your annual deductible.
Copay
A lat fee, separate from deductibles, you pay for certain kinds of
service each time you obtain treatment. Your medical and pharmacy
copayments accrue toward your out-of-pocket maximum.
Out-of-Pocket Maximum
The maximum sum of coinsurance you must pay in a calendar year.
Once you satisfy the maximum, the plan will pay the remainder of any
eligible expenses at 100% for the rest of the calendar year. The out-of-
pocket maximum includes deductibles.
In-Network and Out-of-Network Providers
A network is a group of providers or hospitals that have agreed to
offer medical care at discounted rates. All W. J. Bradley medical plans
offer different levels of coverage, called in-network and out-of-
network, based on whether or not you use providers who are members
of the UHC network.
Prescription Drug Coverage
All W. J. Bradley medical plans include prescription drug coverage. Your
ID cards combine both medical and prescription drug information.
