Enrollment Guide
HEALTHY BY DESIGN
Wellness Program
Whether you are looking to lose weight, get more
active, improve your diet, or simply maintain a healthy
lifestyle, the Healthy by Design program is for you.
When you are healthy you feel better, require less
expensive healthcare and enjoy a better quality of life.
The Healthy by Design wellness program offers you a
variety of services and activities throughout the year.
This includes an online health assessment, educational
opportunities, wellness focused activities, and more.
When you participate in Healthy by Design activities
you earn wellness credits and when you reach the
wellness goals, you will be rewarded! Those who
achieve the wellness credit goal in the current year will
pay a lower medical expense starting the next year in
January versus those who do not. All regular full-time
and part-time partners and employees are eligible to
participate.
Additional activities include:
At-your-desk or view-at-home program online
seminars focusing on nutrition, physical activity,
and disease prevention
Flu shots
Credits for preventive care exams
Free onsite biometric screenings measuring
cholesterol, blood pressure, height, weight, Body
Mass Index (BMI), triglycerides, and blood sugar
levels
The biometric screening, online health assessment,
and one preventive exam are required activities under
the wellness program. When you complete these three To achieve the wellness medical premium rate for
required activities, you will earn 24 credits towards the the year 2022, you must earn 50 total credits. The
due date by which you must earn these credits is
total wellness goal for the year. October 15, 2021.
