SIH Benefits Re-Enrollment

COBRA Continuation of
Coverage


You and your qualiied family members may be

coverage under the plan (e.g., medical, dental and/or
vision) would otherwise end because of a “qualifying
event.” Failure to re-enroll in beneits during the re-
enrollment period is not a COBRA event.

and you will make your decision directly through
