Busey offers three Worksite Beneit Products through Voya.

Critical illness: critical illness insurance pays a lump sum beneit at the
diagnosis of a covered illness. You can choose the level of coverage
you want and use this payment any way you like. Examples of covered

conditions include heart attack, blindness, or major organ failure. Keep
in mind, pre-existing condition limitations and guarantee issue amounts
apply.

Accident: this plan pays a beneit for accidents which occur on or off the
job. Examples of covered accidents include broken bones, eye injuries,
cuts repaired by stitches, burns, and some emergency room treatments.
There is no evidence of insurability required for this beneit. Your spouse

and/or dependents can also take advantage of the accident insurance
program. Your premiums depend on the level of beneit you choose.

Hospital indemnity: hospital indemnity insurance provides beneits for
things such as hospital stays, outpatient surgery, and intensive care unit
stays. You will receive a cash beneit to assist you in these times of need.
The cash can be used however you choose.
