Short Term and Long Term Disability Plan
Summary
ProModel provides full-time employees with Short and Long Term
disability income beneits, and pays the full cost of this coverage. In the
event you become disabled from a non work-related injury or sickness,
disability income beneits are provided to you as a source of income.
You are not eligible to receive Short Term Disability beneits if you are
receiving Workers’ Compensation beneits. Highlights of the plan are
provided below.
Short Term Disability Long Term Disability
Beneits Begin 1st day (accident)/ 91 days
8th day (illness)
Beneits Payable 13 weeks To age 65
Percentage of Income 66.7% of monthly
Replaced 60% of weekly earnings earnings
Maximum Beneit $750 (weekly) $6,000 (monthly)
