Voluntary Life/AD&D
Insurance
TechLab provides you an opportunity to purchase additional optional
group term life and accidental death and dismemberment insurance for
yourself and your eligible dependents.
Coverage is insured by Cigna and is paid 100% by you. Please note
beneits reduce by 35% of the original amount at age 70 and 50% at age
75. Please consult your oficial plan document for additional information.
The cost of this coverage is based on your age and beneit election. Please
contact HR or log onto PlanSource for more information.
Term Life Options
Employee Term Life Options
Maximum Beneit $10,000 increments to the lesser of 5 times salary or
the lesser of $500,000
Guaranteed Issue Limit $100,000—you must complete an evidence of
insurability form if you are electing more than
$100,000
Spouse Term Life Options
Maximum Beneit** A maximum of 50% of the employee’s beneit in
$5,000 increments, up to $250,000
Guaranteed Issue Limit $25,000—you must complete an evidence of
insurability form if you are electing more than $25,000
Children Term Life Options
Maximum Beneit from $1,000 increments to $10,000
6 Months to Age 26
Maximum Beneit from $500
Birth to 6 Months
