At Americare, we recognize our ultimate success depends on our talented

comprehensive program of competitive beneits to attract and retain the Americare
best employees available. Through our beneits programs, we strive to
support the needs of our employees and their dependents by providing a
beneit package that is easy to understand and access and affordable for
all of our employees. This guide will help you choose the type of plan and
level of coverage that is right for you.
With few exceptions, annual enrollment is the only time of year when you
can make changes to your beneits plan.
X Guardian Plans—now effective January 1, 2016
For new employees, your Guardian beneits will be effective on the irst
of the month following 90 days of employment
X Anthem—effective January 1, 2016
For new employees, your Medical beneits will be effective on the 91st
day of employment
X NEW Essential Med Plan—effective January 1, 2016
For new employees, your Medical beneits will be effective on the 91st
day of employment
During annual enrollment you have the opportunity to:
X Add, change, or delete coverage
X Add or drop dependents from coverage
If you have any questions, please contact your facility administrator.
IMPORTANT NOTE: This year, all employees will be enrolling online through the Lockton Marketplace, powered
by beneitsConnect (see page 5 for details). All employees are REQUIRED to enroll in coverage. If you do not
enroll, all beneits that you have currently will end on 12/31/2015. Open enrollment is from November 16 through
December 3, 2015. All beneits will be effective 1/1/2016.
New this year, you will have access to the Ternian Essential Med plan which gives you a less expensive medical
plan alternative that still meets the government-mandate for healthcare coverage.
Guardian has agreed to a one-time guaranteed issue offering for the Critical Illness, Cancer, and Term Life plans.
This means that even if you previously waived coverage, you can enroll without answering any medical questions,
up to the guaranteed issue amount.
