Eligibility and Enrollment
Costs and participation in our beneits program depend on your
employment status and hours worked. Many beneits are fully paid for
by The Polyclinic at no cost to you. Depending on your eligibility to
participate, and your coverage choices, you may pay a small share of
the cost for other beneits.
Coverage for the health and welfare plans begins the irst day of the
month coinciding with, or next following, the date you become eligible.
Eligible employees can begin participation in the 401(k) Proit Sharing
Plan after three months of service.
How to Enroll
1. Visit https://enroll.beneitsconnect.net/polyclinic.
2. Enter your username and password.
A. Your username is up to the irst six letters of your last name,
irst letter of your irst name, and the last four digits of your Social Security
number.
B. Your password is your full (nine-digit) Social Security number without dashes.
3. The online enrollment site will assist you through the enrollment process.
A. Be sure to have your dependent information available during enrollment
(SSNs, birthdates, etc.).
