AD&D Options
You are provided Basic Accidental Death and Dismemberment (AD&D) Insurance by
Franklin Madison—and you may purchase Voluntary AD&D Insurance to supplement your
Company-provided coverage through The Hartford. You pay the full cost of Voluntary
AD&D coverage through after-tax payroll deductions.
Information Online Basic AD&D Insurance
See coverage amounts and costs through the online Franklin Madison provides you with the following
Beneits Portal at www.paylocity.com coverages, at no charge:
y AD&D insurance coverage equal to 1.5 times
your annual Beneit Salary, up to a maximum of
$600,000.
Voluntary AD&D Insurance
Voluntary AD&D Insurance pays a beneit if you are
severely injured or die as the result of an accident.
It covers accidents both on and of the job, at home
and away, 24 hours a day.
You may purchase coverage in an amount equal to
1, 2, 3, 4, or 5 times your annual Beneit Salary, up
to a maximum beneit of $750,000. You pay the full
cost of coverage through pre-tax payroll deductions.
Evidence of Insurability may be required.
You do not have to enroll for Voluntary Life
Insurance to elect Voluntary AD&D Insurance.
If you elect coverage for yourself, you can also
purchase Voluntary AD&D Insurance for your
eligible dependents. Beneits are paid according
to your family makeup, as shown in the following
chart.
