Qualifying Life Event

A qualifying life event is a change in your personal life that may impact
your eligibility or dependent’s eligibility for beneits. Examples of some
qualifying events include the following.


„ Change of legal marital status (e.g., marriage, divorce, death of
spouse, legal separation)

„ Change in number of dependents (e.g., birth, adoption, death of
dependent, ineligibility due to age)

„ Change in employment or job status

Benefits Eligibility

Eligible Employees

You may enroll in the beneits program if you are a regular full-time
employee who is actively working a minimum of 30 hours per week.


Eligible Dependents

As you become eligible for beneits, so do your eligible dependents. In
general, eligible depends include your legally married spouse* and children
up to age 26 or age 30 if you are an employee who resides in the state of
Florida. Children may include natural, adopted, step-children, or children

obtained through court-appointed legal guardianship.

SSN Required

You must provide a valid Social Security Number for yourself and each
enrolled dependent. Employers are required to provide names and Social

Security Numbers to the federal government for each individual enrolled
for medical coverage. This reporting requirement helps the federal
government verify compliance with the Health Care Reform Individual
Mandate.

* If you have a spouse who works and he or she is offered medical beneits through his or her
employer, they must elect coverage through their employer .
