Rates for this plan are available through the enrollment portal as they vary based on age, tobacco use and
beneit amount selected.

Voluntary Short Term Disability

Coverage



Starting in 2017, coverage will no longer be available for voluntary short term
disability (STD) through Colonial Life. However, for those individuals who are
currently enrolled, coverage can be continued but on a direct basis. Colonial Life will
provide directions how to retain coverage and submit premium. Premium rates are

portable and will not change when moving from payroll deduction to direct payment.
