Critical Illness
Alac Group Critical Illness insurance can help with the treatment costs of covered critical illnesses, such as
cancer, a heart attack, or a stroke. More importantly, the plan helps you to focus on getting better (instead of
the distraction and stress over the costs of medical and personal bills). Alac Group Critical Illness insurance
provides a lump-sum beneit and gives you the lexibility to help pay bills related to treatment or to help with
everyday living expenses. Please refer to your Alac beneit summary for a detailed outline of coverages.

upon separation from CSA Service Solutions, LLC provided you contact Alac and apply within 31 days of your
termination date.
