Eligibility For Benefits Important Deadline
Covering Yourself Reminder
You may enroll in the beneit plans if you are a regular If you experience a qualifying life event, you

Covering Your Family Members
Eligible dependents include your legally married spouse
and children up to age 26 for medical, dental, and
vision coverage. Children include natural, adopted,
step-children, as well as children obtained through
court-appointed legal guardianship. Children will be
covered until the end of the month they turn 26.
Disabled dependents may be eligible beyond 26. It is
your responsibility to notify Human Resources when a
dependent is no longer eligible for beneit coverage.
Changing Your Benefits Mid-Year
Once you make your elections, you will not be able
to make changes until next annual enrollment unless
you experience a qualifying life event. Examples of
qualifying events include the following.
Change of legal marital status (e.g., marriage,
divorce, death of spouse, legal separation,
annulment)
Change in number of dependents (e.g., birth,
adoption, death of dependent, ineligibility due to
age, court issued qualiied medical child support
order)
Change in employment or job status for you or one
of your dependents
Your spouse’s employer’s medical plan, in which you
are currently enrolled, has an open enrollment
