Life Insurance
Basic Life Insurance
Intertek provides basic life insurance with accidental death and
dismemberment insurance at no cost to you. This coverage offers inancial
protection to you if you suffer certain debilitating injuries or to your
beneiciaries in the event of your death. If you die while covered by this
plan, the beneit is paid to the beneiciary (or beneiciaries) you designate,
in writing.
Basic Life Benefit
X Two times your base annual earnings up to a maximum of $500,000
X A lat payment of $5,000 for your spouse or same-sex domestic partner
X A lat payment of $2,000 for dependent children
Voluntary Life Insurance
If you would like to purchase supplementary life insurance in addition to Age Band Rate
your company paid beneits, Intertek offers you the ability to purchase 18-29 $.06
voluntary life insurance at competitive group rates. Voluntary Life Insurance 30-34 $.06
can be purchased from 1 to 6 times your base annual salary.
35-39 $.10
Evidence of Insurability (EOI) or proof of good health is required under the 40-44 $.14
following circumstances: 45-49 $.22
50-54 $0.37
X Late entrant: you have previously waived the opportunity to elect this 55-59 $.59
coverage and are now electing coverage for the irst time 60-64 $.73
X Current participant: you currently have this coverage and are 65+ $1.16
requesting an increase to your current coverage amount
X Newly eligible: you have never been offered this coverage previously Optional life insurance rates for you
or waived this coverage previously and you are requesting over the and your spouse or same-sex domestic
guaranteed issue amount partner.
You can also purchase voluntary life coverage for your spouse or same-
sex domestic partner if you’ve also purchased voluntary life coverage for
yourself. This coverage is available in $10,000 increments not to exceed
the lesser of 50% of your employee election or $120,000.
