Supplemental AD&D
Enrollments must
be made online at Employee Rates
www.portal.adp.com You may elect Voluntary AD&D coverage in multiples of you base

annual earnings. You may elect 1, 2, 3, 4, or 5 times your base annual
earnings. The maximum amount is $500,000 and the minimum amount
is $10,000. If you elect coverage for yourself, you may also elect coverage
for your dependents.


Spouse and Child(ren) Rates

If you apply for Voluntary AD&D coverage for yourself, you may also
elect coverage for your spouse and dependent children. The coverage
amount for each dependent is as follows.


“ Spouse only: 60% of employee’s Voluntary AD&D coverage amount
“ Children only: 20% of employee’s Voluntary AD&D coverage
amount

“ Spouse and children: 50% of employee’s Voluntary AD&D coverage
amount for the spouse and 10% of employee’s Voluntary AD&D
coverage amount for each child
