Life and Disability Coverage 2017 Beneits Enrollment
Life and Accidental Death and Dismemberment (AD&D)
Insurance Coverage
Life insurance is an important part of your inancial security, especially if others
depend on you for support. Accidental death and dismemberment (AD&D) insurance
is designed to provide a beneit in the event of accidental death or dismemberment
(loss of limb or paralysis).
F+W provides basic life and AD&D insurance to all eligible employees at no cost to
you. You are responsible for the optional life/AD&D insurance premiums if elected.
X Amount of basic life insurance = $50,000
X Amount of basic AD&D insurance = $50,000
X Supplemental life insurance for employees is available in $10,000 increments subject
to a maximum of 5 times the basic annual earnings or $500,000 whichever is less
X Supplemental life insurance for spouses may be elected in $5,000 increments up to
50% of the insured employee’s supplemental life election
X Supplemental life insurance for children may be elected in a lump sum of $10,000
or $20,000 for child(ren) from birth to age 19 or 25, if a full-time student
X Supplemental AD&D Insurance is available in the following increments
Z Single—$100,000, $175,000, or $250,000
Z Family—$100,000, $175,000, or $250,000
Evidence of Insurability
If it is outside of your new hire eligibility period, any change to your election will
require evidence of insurability (EOI). EOI is an application process in which you
provide information on the condition of your health, or your dependent’s health
in order to be approved for life insurance coverage. EOI is required for any late
entrant or current participants who wish to increase their coverage amount. All EOI
applications are subject to AIG’s review before they are considered inal.
