Life, Voluntary Life, and EAP
Basic Life Insurance
Premier Performance Products provides, at no cost to you, basic life and accidental
death and dismemberment insurance for all employees working 17.5 or more hours
per week. In the event of your death, our life insurance policy helps provide a
general safety net for your beneiciaries. Your coverage is equal to $10,000 in life
insurance and $10,000 in AD&D insurance. If your death is the result of an accident
or if an accident leaves you with certain debilitating injuries, you will be covered
under our accidental death and dismemberment insurance for the same amount. We
hope this company paid policy helps you feel more secure and prepared to manage
your inancial goals and obligations.
If you die while covered by this plan, the beneit is paid to the beneiciary (or
beneiciaries) you designate in writing. This core life insurance coverage is paid in full
by Premier Performance Products and is fully insured by Unum.
Voluntary Life Insurance
We recognize you and your family have speciic needs and our company paid life
insurance policy may not be enough to ensure your inancial security if your income
were suddenly lost. If necessary, we offer a voluntary life insurance option with
competitive group rates so you can purchase and secure the inancial protection you
need. Voluntary life insurance can be purchased in increments of $10,000 up to ive
times your basic annual earnings to a maximum of $500,000.
Spousal life insurance is also available in increments of $5,000 up to 100% of the
employee life amount or $500,000
You can also insure you dependent children for a lat amount of $10,000.
Evidence of insurability (EOI) or proof of good health is required if you select
more than the guarantee issue of $110,000 for employees and $25,000 for spouses.
When you purchase voluntary life coverage for yourself, your spouse and/or your
children, you will automatically be enrolled in voluntary AD&D coverage as well.
