Basic Accidental Death and Dismemberment
(AD&D) Insurance
If your death is the result of an accident or if an accident leaves you with
certain debilitating injuries, you’ll be covered under our accidental death
and dismemberment insurance for the same amount as the basic life
insurance beneit.
AD&D Insurance At-a-Glance
Prudential
Eligibility All active, full-time employees
AD&D Insurance-All Guaranteed Coverage
Basic Employees receive 1.5× annual salary up to
a maximum of $250,000
Voluntary $10,000 increments to a maximum of 7×
your earnings to a maximum of $500,000
Dependent Voluntary AD&D
Spouse $5,000 increments to a maximum of 50%
of the amount of coverage you elect for
yourself to a maximum of $250,000
Child (children from live birth to 26 $2,000 increments to a maximum of 50%
years are eligible) of the amount of coverage you elect for
yourself to a maximum of $10,000
Long Term Disability—
Prudential
Metropolitan Development and Housing Agency of America, Inc.
provides long term disability (LTD) beneits to you through Prudential,
with you paying 50% of the cost. You will be automatically enrolled on
the irst of the month following your date of hire.
Long Term Disability (LTD) Benefits
When Benefits Begin and End
LTD income beneits begin after you have been disabled for 180 days.
Beneits end the earlier of the date you are deceased, the date you cease
to be disabled, or when you have reached the maximum duration schedule
relected in the SPD. You are not required to be totally disabled during the
180 day elimination period.
How to File a Claim
If you have questions about LTD insurance or need help iling a claim,
please contact your Human Resources department.
