Providers and pharmacy searches
Prescription drug formulary
Access to temporary ID cards and means to order another ID card
Information regarding paid and pending claims
Health and wellness information
You can search for participating providers on the [Carrier] site,
www.[Carrier].com. Complete the following steps.
1. Go to [Carrier].com
2. Click on “Find a Doctor” on the left side of the screen
3. Click “Provider Finder”, then select the state in which you live
4. Choose “PPO (Participating Provider Options) for network type”
5. Enter additional search criteria
