beneits to make sure your trip to the eye doctor is reasonably priced.
Guardian offers associates comprehensive vision coverage through the Avesis network. Keep in mind, the

Bifocal Lens $25 copay Amount over $50 a program for managing hearing care and obtaining brand
Trifocal Lens $25 copay Amount over $65 name hearing aids through a national network of preferred
Frames providers.
Frame Beneit $130 allowance Amount over $70
then 20% discount
on amount over Participant’s simply call EPIC at 866.956.5400 to start the
allowance process.
