A qualifying event is a change in your personal life which may impact your eligibility
or your dependent’s eligibility for beneits. Examples of some qualifying events

include the following.

X Change of legal marital status (e.g., marriage, divorce, death of spouse, legal
separation)

X Change in number of dependents (e.g., birth, adoption, death of dependent,
ineligibility due to age)

X Change in employment or job status

If your status changes from part-time to full-time, you are eligible for beneits the
irst of the month following the date of the status change. It is your responsibility to
notify Human Resources within 30 days of the qualifying life event. Failure to notify
Human Resources within 30 days will prohibit your ability to change your beneits
election(s).



Employee Eligibility

Eligible Employees

ClearChoice’s full-time employees working 30 hours or more per week are eligible for
our health, dental, vision, life, and disability plans. Newly hired employees become
eligible on the irst of the month following 30 days of employment.


Dependent Eligibility

As you become eligible for beneits, so do your eligible dependents. In general,
eligible dependents include your spouse and children up to age 26. If your child is
mentally or physically disabled, coverage may continue beyond age 26 once proof
of the ongoing disability is provided. Children may include natural, adopted, step-
children, or children obtained through court-appointed legal guardianship.
