Eligibility and Enrollment Online Enrollment


To enroll or decline your

An Eligible Employee is: medical, dental, HSA,
Dependent Care FSA, Life or
An employee scheduled for 30 or more hours per week, or LTD benefits login to ADP
located on:
An employee who gains eligibility by meeting Affordable Care
Act guidelines. Please see HR for more details or to discuss Mission Control or
your eligibility. https://workforcenow.adp.com

An Eligible Dependent is Considered to Be:
Your legal spouse, an individual to whom you are legally
married.

Your “Qualified Domestic Partner”, as defined in Section 14,
Glossary; there is a 12-month waiting period after the date
in which a domestic partnership is terminated before a new
eligible partner can be enrolled*;

Your child, or your spouse’s or domestic partner’s child who
is under age 26, including a natural child, stepchild, a legally
adopted child, a child placed for adoption or a child for whom
you or your Spouse are the legal guardian; or

An unmarried child age 26 or over who is or becomes disabled
and dependent upon you.

To be eligible for coverage under the plan, a dependent must reside
within the United States.

*Please contact Human Resources to complete a domestic partner
certification (here) if you are enrolling a domestic partner.
