Wellness 2016–2017 Beneits Enrollment
RallySport Direct is excited to offer Boost Your Health, a wellness program through
Vasa Corporate Fitness.
If you wish to participate in the program, please complete the Boost Your Health
enrollment form. This enrolls you in the program until next year’s annual enrollment
period.
If you sign up for this program, the company will pay $30 per month towards the
cost. The remaining $10 per month fee is payroll deductible from your paycheck.
This provides you a VIP gym pass, along with the opportunity to earn points by
doing various activities such as going to the gym, meeting with a wellness coach, and
reaching monthly personal goals.
If you achieve 600 points or more in a quarter, you will qualify for a $50 discount on
your insurance premium for the next quarter.
The following is an outline of the points awarded for each activity. Goal is a total of
600 points per quarter.

3 Healthy Nutrition Choices Recorded in UPLIFT 5
Nutrition Tracking—If Participant is Working with a Health Coach 5
They Can Earn Points for Tracking Food Each Day
Note: challenge points vary. Company event points can be customized. Changes to the points system could be
made and would be announced at the beginning of each quarter.
