Benefits Eligibility ENROLLMENT DETAILS
Covering Yourself Annual Enrollment
You may enroll in the beneits
programs if you are a regular, Annual enrollment is your yearly opportunity to review your current
full-time employee who is actively beneits and make beneit changes for the upcoming plan year. During
working a minimum of 30 hours this annual open enrollment, you can add, change, or decline coverage. In
per week. addition, you can add and/or drop dependents during this time. Outside
of annual enrollment, you cannot change your beneit elections. During
Covering Your Family the middle of the plan year, you may only change your level of beneits
Members coverage if you experience a qualifying life event.
Eligible dependents generally
include your legally married Qualifying Life Event
spouse and children up to age 26. A qualifying life event is a change in your personal life or employment
Age limitations vary by program. which may impact your eligibility or dependent’s eligibility for beneits.
Children may include natural,
adopted, step-children, or children Examples of some qualifying life events include the following:
obtained through court-appointed z Change of legal marital status (e.g. marriage, divorce, death of spouse)
legal guardianship.
z Change in number of dependents (e.g. birth, adoption, death of

Please be prepared to share z Change in employment or job status (including shift from part-time to
dependent eligibility information full-time or vice versa)
during enrollment including z Termination or commencement of your spouse’s coverage in general
date of birth and Social Security when coverage is maintained through the spouse’s plan
Number. Other documentation z A resident or worksite change which impacts healthcare
may be required depending on

your qualifying event. If you do not make changes during this time, you
may have to wait until next year’s annual enrollment to make your change.

next beneits enrollment period.
