Your 2020 Benefits Guide
Your management team at American Oncology Network is extremely
committed to your well-being and satisfaction with the group beneit
plans. Full-time employees scheduled at least 30 hours/week and part-time
employees scheduled between 24–29 hours/week are eligible for coverage on
the irst day of the month following 60 days of continuous employment.
Your major medical is being offered through Blue Cross Blue Shield. The
retail pharmacy beneit manager is Prime Therapeutics, and the Specialty
medications is through CVS Caremark and Alliance Rx Walgreens Prime.
Dental, Life, and Disability coverage are offered through Cigna. Vision is
offered through EyeMed.
The Medical, Dental, and Vision beneits offered by the company are covered
under the IRS Section 125 Plan. This plan allows your premium contributions
to be taken out of your paycheck before taxes are applied. This results in a
greater take-home pay for you.
Because your share of the plan cost is deducted from your paycheck on a
pretax basis, the IRS permits changes to your elections only when a family
status change takes place or during open enrollment. Examples of family
status change include marriage, adoption, birth, divorce, and your spouse
obtaining/losing other group coverage (through his/her employer).
If you would like to make a plan change due to one of the listed reasons,
you must contact Human Resources within 30 days following the event.
Otherwise, no changes will be permitted until the next annual open
enrollment. Remember, if you change your beneit elections, your beneits
