Life and Accidental Death and Dismemberment (AD&D)
Our life and accident insurance plans provide inancial protecion in the event of the death of a covered person or
upon certain other losses sufered as the result of an accident. Our company paid Life and AD&D plan consists of basic
coverage provided at no cost to you and supplemental insurance you may elect if you wish to have addiional coverage.
Basic Life And AD&D Insurance
Beneit Amount 2x annual salary to a max of $350,000, Senior Level Execuives have a maximum of $1,000,000
Supplemental Employee Life Insurance
Beneit Amount Up to 5 imes salary or a maximum of $1,000,000
Beneit Increments $10,000
Guaranteed Issue Amount $250,000
Supplemental Spouse Life Insurance
Beneit Amount Up to 100% of the employee elecion or $500,000, whichever is less
Beneit Increments $10,000
Guaranteed Issue Amount $50,000
Supplemental Child Life Insurance
Beneit Amount $10,000
During your Iniial Enrollment period you can select any coverage amount with no quesions or health exams up to
the guaranteed issue amount of $250,000. This Iniial Enrollment period is the only ime you can get this guaranteed
coverage. The spousal guaranteed coverage* up to $50,000 is only available during the Iniial Enrollment Period.
During future enrollments, you can increase your coverage (employee and spouse), but will need to answer health
quesions.
* Medical quesions not required, but Spouses are subject to certain eligibility requirements (ex. spouse cannot be fully disabled at ime of elecion).
You will be asked to provide beneficiary
information when you enroll online.

reflect your annual salary as
of your hire date.
