your costs for the year, all based on your unique situation.
To Get Started
X Go to healthpartners.com/planforme
X Enter 19886 for the group number, 0 for the site number, and 01/01/2017 as the
effective date
X Enter age and gender for all covered members under the plan
X Pick from a list of common conditions and medical procedures you think you
might get
X Plug in any medications you are taking
X Check to see if your doctor is in-network
X Print out a summary or email it to yourself
