As shown in the table on page 8, all employees who earn 50 points will Call CHC Wellness at
866.373.4242
qualify for a $50 pre-paid gift card. In addition, employees who are
enrolled in the medical plan who earn 100 points will qualify for a $100
gift card and the wellness medical contribution rates for the entire 2017
calendar year. Those with accumulated points between two levels will
earn the lower level incentive.
Outside of the health risk assessment and biometric screening there
are various ways you can learn more about your health and earn points.
Below are additional programs and resources available to you through
CHC that can help you stay committed and healthy all year long.
Health coaching: a personal health coach will help you review your
blood work and health risk assessment results; you can then work
with your coach to set your own wellness goals
e-Learning: your CHC Wellness personal portal gives you access
to thousands of e-learning courses; topics range from prenatal
care to diabetes prevention and cholesterol management; for
basic education or to address a speciic problem area, these quick
courses can help you stay on track
Incentive tracking: along with your follow-up programs, you can
earn points; just login to your personal wellness portal and record
your participation, then watch the incentive points add up in real
time
