Vidant Health Benefits Guide




Disability



The disability beneits consist of a Short-term Disability (STD) Plan, Long-term Disability (LTD) Plan, and an
Extended Illness Bank (EIB) for certain eligible team members. The plans work together to replace a portion of
your income when you are on an approved absence from work for your own serious health condition. Only for
Annual Enrollment 2021 you can select a disability plan without submitting an Evidence of Insurability (EOI).

Extended „ Full-time and part-time members receive
Illness Bank a beneit for income replacement

„ Income replacement at 100%

Short-term „ Two plan options available: 15/11 and
Disability 30/09
(STD) „ 15/11 has a 15-day elimination period
and pays up to 11 weeks of beneits
„ 30/09 has a 30-day elimination period
and pays up to 9 weeks of beneits
„ Pre-existing conditions apply*
„ Contact Lincoln Financial to ile your STD
claim at 800-213-3805
„ Replaces 50% of your pre-disability pay
($1,000 per week maximum)
Long-term „ Provides coverage after a 90 day
Disability elimination period
(LTD) „ Replaces 50% of your pre-disability pay
($5,000 monthly maximum)
„ Pre-existing conditions apply*

claim: 800-213-3805
* NOTE: If you become disabled during the irst 12 months of coverage
due to a pre-existing condition, the Long-term Disability plan may not
pay beneits.

Your effective date of coverage is the irst day of the
month following 30 days of employment or the irst
day of the month following 30 days of transferring into
a beneit eligible classiication.
