Short Term Disability



DAWSON provides a salary continuation plan if you are disabled and unable to
work. Please refer to DAWSON’s employee handbook and STD policy for details
on the beneit and how to ile a claim.

Voluntary Long Term Disability


The company offers voluntary long term disability insurance to provide inancial
security should you be out of work for an extended period. Disabilities lasting
more than 90 days are considered “long term.” In the event you are unable to work
due to any accident or illness, this program will pay you a monthly beneit of 60%
of your pre-disability monthly earnings to a maximum of $10,000 per month.
Upon approval, beneits would begin on the 91 day and would continue until you
st
are no longer disabled or until you reach the Social Security normal retirement age.
The plan has pre-existing condition limitation exclusion. A pre-existing condition
includes any condition/symptom for which you consulted with a physician,
received treatment, or took prescribed drugs in the 3 months prior to your effective
date under plan. If you become disabled within the irst 12 months of coverage
due to a pre-existing condition, no beneits would be provided by the plan. Please
review your enrollment materials in ADP/WFN for more plan details.


* If you did not elect coverage when initially eligible, you will be required to provide proof of
insurability to elect coverage during this open enrollment period.
