Eligibility and Enrollment Qualifying Events

As a new employee at The Polyclinic, it is important that you review the status
beneit options available to you as this is your irst opportunity to elect It is your responsibility to notify the

4. Once you’ve completed the online enrollment process, be sure to print your
elections for your personal records.

If you waive beneits at this time the next opportunity will be at open enrollment.
