Basic and Voluntary Life insurance Basic and Voluntary Life
is offered through The Hartford .
Life Insurance (NextCare Paid Benefit)
NextCare provides a life insurance beneit in the amount of one times
your annual salary to a maximum of $250,000 (subject to age limitations).
This coverage comes with an accidental death and dismemberment beneit.
NextCare pays for the premium of this beneit, therefore you have no payroll
deduction. Please elect a beneiciary for this plan. A full summary of this
beneit can be found on the NextCare Intranet.
Voluntary Life Insurance
Supplemental life insurance is offered to employees, which gives you the
opportunity to buy life insurance coverage for yourself, your spouse, or your
dependent children in addition to the life insurance NextCare provides.
Employee Benefit
$10,000 increments to a maximum of ive times your annual salary (cap
$500,000 and subject to age limitations).
Dependent Benefits
Once you elect coverage for yourself, you can elect coverage for your spouse
and/or children.
X Spouse—$5,000 increments (cap $250,000 and subject to age limitations)
X Children 2 weeks to 6 months—$100
X Children 6 months to 26 years (if student)—$1,000, $2,000, $4,000,
$5,000, or $10,000
The maximum the employee can purchase is $150,000 without having to
complete a Personal Health Application and the maximum the employee
can purchase for spouse coverage is $30,000 without having to complete a
Personal Health Application. An employee can elect Voluntary Child Life
without electing Voluntary Spouse Life. The Voluntary Spouse or Voluntary
Child amounts elected cannot exceed 50% of the Voluntary Employee
beneit amount. The cost of this coverage is determined by your age. Please
