exceptional care of you. This guide briely summarizes the programs and resources available to NextCare
employees and their family members. Your beneits package is one of the ways we recognize how important you
are to NextCare and the work we do. Thank you for all you do for our patients or in support of our patients.
NextCare provides pre-tax payroll deductions per the IRS code Section 125 Plan per our Premium Only Plan written by BASIC for
medical insurance, dental insurance, vision insurance and health savings account contributions . If you do not indicate otherwise,
your payroll deductions will be on a pre-tax basis . Short term disability, voluntary employee/spouse/child life insurance, critical
illness, accident, legal and identity theft plans are on a post-tax basis . Once making your benefit selections, you are limited to
the changes you can make unless you have a qualified “life event” during the plan year or during the next annual open enrollment
period . Please see the Mid-Year Enrollment Changes section of the Guide for details on qualified life events .
