Life Insurance

Basic Employee Life Insurance and Accidental Death and
Dismemberment (AD&D)

Life insurance helps ease the inancial burden your dependents may face in
the event of your death. W. J. Bradley provides Basic Term Life Insurance
and Accidental Death and Dismemberment (AD&D) coverage at no cost
to you. Coverage is automatic—you do not need to enroll in this beneit,

however, make certain to name your beneiciaries online.

The life insurance beneit pays $25,000 in the event of a qualiied
employee’s death.


In addition, deaths and dismemberments due to accidents may pay up to an
additional AD&D beneit of $25,000. Life and AD&D insurance beneits

will reduce by 35%, 60%, and 75% of the original amount at ages 65, 70,
and 75, respectively, and will terminate upon retirement.
