Short Term Disability

You are eligible to purchase short term disability through Humana.
If you are working at a full-time job while coverage is in force and
a covered off-the-job injury or illness causes you to become totally
disabled, Humana will pay the following beneits.

Disability Coverage
Beneit percentage maximum 60% of base monthly income
Monthly beneit minimum $300
Monthly beneit maximum $3,000
Elimination period—accident 14 days
Elimination period—sickness 14 days
Beneits duration 3 months
Pre-existing condition exclusion Any medical condition which has been
diagnosed or for which you have received
treatment during the 12-month period before
your initial effective date will not be covered
until 12 months following your effective date.
This includes pregnancy.

Plan Details

You can purchase up to a monthly beneit maximum of $3,000 on a
guaranteed issue basis. No medical questions will be asked.

are based on your current
