offerings for Area Supervisors, Store Managers, Assistant Store
Managers, and home ofice based employees. This guide is a general
overview of our plan offerings with high-level information. To
see more detail and view the beneits that are speciically available
to you at this Annual Open Enrollment, please visit our employee

Annual Enrollment
We encourage you to go online and review your beneit elections
and your beneiciary information. Your changes must be made by
December 15.
You are eligible to enroll in the following plans:
Medical plan (PPO) Term life and AD&D
Dental Accident
Vision Short term disability
Note: these plans are not dependent on each other. You may choose
to enroll in any of the plans outlined in this guide.
MotoMart’s Annual Enrollment Period will be
December 4 to December 15.
