Short Term Disability
„ Provides you the opportunity
to protect your income in the
event of a covered disability
„ Coverage is available in
amounts up to 60% of eligible
income to a maximum of
$5,000 per month
„ Beneits paid to you are
tax-free because you pay
the premium
„ You have the ability to choose
the elimination period which
best meets your needs
„ You own the policy and rates
are based on age at issue and
will not increase due to age

Critical Illness Insurance
„ Provides lump sum beneits at the diagnosis of a covered illness;
additional conditions are included for dependent children
„ Beneits can be used to help employees manage the inancial impact
of expenses which may not be covered by medical insurance
„ Wellness beneit included which pays insured employees and their
covered dependents $50 annually when they receive a covered
health screening
„ Coverage is portable and employee beneit amounts up to $50,000
are available
Accident Insurance
„ Designed to help you meet those out-of-pocket expenses which can
follow even ordinary accidents
„ Can help provide inancial protection against the cost associated with
deductibles, copays, travel expenses, etc.
„ Beneits are paid directly to you; you choose how to spend the money
„ Covers you 24 hours a day, both on and off the job
„ Wellness rider pays a $50 beneit per calendar year, per covered
individual for a covered health screening test which is performed
„ You can purchase an optional rider which can pay you $200 per day
when conined to a hospital due to a covered sickness
„ Coverage is 100% portable if you leave work
