Vision Plan—VSP
First Bank’s vision beneit is offered through the VSP Signature Choice
Plan. For a small per pay period contribution, participants receive
signiicant discounts on an array of eye care services. Because some
beneits are provided on an every other year basis, employees are required
to participate on a two-year schedule.
VSP Summary of Benefits
Service Beneit
Well vision exam $10 copay
Prescription glasses $25 copay
Lenses (every 12 months) Single, bifocal, and trifocal; polycarbonate for
dependent children
Frames (every 24 months) $150 allowance on frame of choice; 20% discount
on amount over allowance
Contact lenses (every 12 $150 allowance on contact lenses and contact lens
months)* exam
Other discounts 20% off lens options such as: progressives, scratch
resistant and anti-relective coatings; 15% off
regular price of laser vision correction services
* Contact lens coverage is provided in lieu of prescription glasses
