Disability and Travel
Long-Term Disability (LTD) Insurance
Long-term disability (LTD) insurance provides inancial protection by
paying a portion of your income while you are disabled. Full-time associates
will be provided LTD coverage in the group plan through Reliance Standard.
Disability payments begin after 90 days from the date you are deemed
disabled. The payment beneit is 66 / 3% of your monthly earnings as

determined under the policy to a maximum of $10,000 per month. The
beneit can continue until the date you return to work or the latter of age
65 or your Social Security Normal Retirement Age (SSNRA).
Business Travel Accident
ARCO/Murray has an accidental death and dismemberment policy through
Chubb to cover you while you travel on business. The beneit is $250,000
and is at no cost to you.
Important Note
Now is your opportunity to add or drop you and your dependents from
medical, dental and/or vision coverage. Additionally, now is the time to
make any changes to your supplemental life insurance * or FSA elections.
Please contact Human Resources to make changes to your coverage.
If you do not make changes at this time, your current plan elections will
remain in place and employee contributions will be adjusted accordingly.
Remember: FSA elections must actively be made each year. The IRS
does not allow FSA elections to automatically default to the previous years
election.
* Evidence of Insurability (a health questionnaire) may be required.
