               Please be sure to read the important information below:

               A spouse is eligible for the W International health insurance plan only if the spouse is not
               eligible for health insurance through the spouse's employer, Medicare, or the military.


               Discuss the plan options with your family and determine what best fits your needs. All changes
               must be made by completing the 2024-2025 Online Enrollment even if you are declining
               coverage. If you are electing voluntary disability or voluntary life insurance, you may also have
               to complete the evidence of insurability form.


               You must submit a completed Evidence of Insurability Form (EOI) if you are an existing
               employee electing Voluntary Life and AD&D for the first time or if you wish to elect above the G
               uarantee Issue for an existing Voluntary Life and AD&D Policy.

               If you are a new employee, you may elect up to the Guarantee Issue (GI) amount without
               having to provide an EOI form. Completed EOI forms must be turned in to the HR Department
               for your request to be considered.

               Please Note: Failure to provide a completed EOI form will result in denial of request.
               Open enrollment is the period of time when employees may elect or change their benefit
               options.  This is the only time you can make such changes unless you have a qualifying life event
               (marriage, birth of child, etc.) that triggers a special enrollment period. The 2024-2025 Open
               Enrollment will be passive meaning you are re‐enrolled with your existing plans to avoid any
               coverage lapses as a result of NO ELECTION for Medical, Prescription, Voluntary Dental,
               Voluntary Vision, Voluntary Life and AD&D and Voluntary Short & Long-Term Disability, etc.


                If you are changing plans, please be sure to notify your doctor’s office and pharmacy of your
                new insurance plan and give them your new insurance card.
