5 . DETAILS OF TRAIN ACCIDENTS ON HYB DIVISION FOR CURRENT YEAR

(2020-21): 01

Particulars:

a) Yard & Misc. Accidents during the current year 2020-21 : One

On 03.11.2020, at 13.45 hrs., Fire occurred in WGSCN-05275 coach (last coach towards MOB
end), in CBC coaching stock which was stabled on the MMTS-1 spur of MEDCHAL station.
Cause: Miscreant activity.
(JAG officers committee consisting of Sr.DSTE(Sr.DSO), Sr.DOM, Sr.DSC and Sr.DME conducted
inquiry on 06.11.2020).

Staff responsible:

Primary: Nil
Secondary: Nil
Blame worthy:

1) Sri Farooq pasha, JE/C&W/KCG : issued with „Warning letter‟ on 04.12.2020.
2) Syed Iqbal Ahmad, IPF/MJF: „No action‟ decision initiated by Sr.DSC.
3) B. Venkatesh, Pointsman/MED: issued with „Warning letter‟ on 15.01.2021.

b) Consequential Train Accidents, Other Train Accidents and SPAD during the current
year 2020-21: NIL.

6. DETAILS OF TRAIN ACCIDENTS ON HYB DIVISION FOR 2019-20, (PREVIOUS YEAR)

a) Other Train Accidents : One.

On 09.10.19 at 16.55hrs. at MQN station, BCM Machine 289 while proceeding from MQN (Loop
line) to MBNR, taken two routes at point no. 7 and rear trolley derailed in sand hump blocking both
Main line and Loop line.

(Cause: Station Master failed to ensure complete movement of un coupled machines before
operating point No.7)

The following staff were held responsible:

Primary: P.E.Prasanna kumar, SS/MQN (SS/MBNR on duty @ MQN)(Issued with a Major penalty
of “Removal from service” on 07.02.2020.)
Secondary: Nil
