Risk Assessment – 42Q – Working at Height and the use of Ladders..

This record is to be kept on the driver's file.
To assess the understanding of this risk assessment and the safe system of work (RASSoW) the driver carrying out this assessment must answer the questions correctly to
show that they have understood what is required to carry out this task safety. Any questions answered incorrectly will be explained in more detail by the person rolling
out this training. This is not a test. It's an assessment. Read the question and place A, B, C or D in the white box.

Question 1 – Working at Height and Ladders. Drivers can NOT use: Question 5 – Falls from Height. Some driver Conditions that may make you more
likely to fall may include:
A. Company Vehicles. A. Wearing your safety boot.
B. Ladders. B. Wearing your high visibility vest.
C. Tablets when identifying jobs. C. Difficulties with walking and balance.
D. Indicators when turning left. D. Not knowing the direction to walk.

Question 2 – Working at Height. Climbing: Drivers are to remember that they MUST Question 6 – Falls from Height. Many falls from height occur when workers:
NOT climb on or up the sides of either their vehicle, the trailer and/or:
A. Slip.
A. Skips and Bins.
B. Get hungry.
B. Mountains.
C. Use a safety harness.
C. PPE Vest and hat.
D. Wear not slip boots with a steal toe cap.
D. Rings on their fingers.
Question 3 – Three points of contact. Drivers are reminded that it is good practice to Question 7 – Fall Prevention. If you do fall or gain an injury, then you must complete
look down before taking the final step to: the Assure app and:
A. Ensure you have your PPE on. A. Call Lincs Commercials.
B. Inform the transport office.
B. Complete a defect report.
C. Cover the trailer. C. Park the vehicle up. RA-42Q
D. Complete a defect report.
D. Identify what you are standing on when exiting the vehicle.
