18. I understand that policies regarding evaluation will be set out in each course
syllabus and may include but not limited to: examinations, quizzes, skill checkoff,
role playing, oral presentations, written papers, demonstrations, participation in
field trips, and attendance at CE and group projects. _____
19. I understand and will comply with the appropriate dress code for each academic
and clinical situation. _____
20. I will comply with personal hygiene and grooming requirements of this program
and recognize that failure to do so could lead to dismissal from the program.

21. I will not use any form of tobacco/vapors/e-cigarettes on MSC campus. _____
22. I understand the policies and procedures pertaining to re-admission to the
Veterinary Technology program. _____
23. I understand I will may have to provide my own transportation and/or housing
during my externship. _____
24. I understand the importance of maintaining confidentiality in all clinical situations
and understand that violating confidentiality by discussing any patient
information outside clinical setting or with anyone other than my clinical
instructor or academic faculty may result in my dismissal from the program.
